Criteria 1: Lived in a Disproportionately Impacted Area (DIA) for 5 of the last 10 years (2014-2023)- Does Not Have To Be Consecutive
Provide: TWO DOCUMENTS validating Your Full Address, Residence Type (Lease/Rent, Own, Public Housing), and Move-in and Out Dates.
